  • The foundation of clinic P&Ps is the P&Ps manual—a series of documents that describes the clinic’s day-to-day functioning and outlines its P&Ps.
  • Clinics need a standard format for all P&Ps. A typical format begins with the policy purpose, continues with the policy statement, and concludes with a step-by-step description of the procedures required to implement the policy.
  • At minimum, P&Ps for the following should be in place from the time a clinic opens: organizational structure; scope of services; hiring, including credentialing and privileging of professional staff; financial management; clinic insurance; appointment and walk-ins; informed consent; infection control; hazard communications; clinic safety; and evaluation.
