The appendix contains supporting documents, such as

  • Resumes of key staff.
  • Audited financial reports for 3 years.
  • The clinic budget.
  • Studies or statistics that support financial assumptions.
  • Studies or statistics that support the market analysis.
  • Newspaper articles that highlight the clinic.
  • Examples of marketing literature.
  • Maps of service area(s) and clinic location(s).
  • Organizational chart.
  • Photographs of clinic(s).
  • Drawings of proposed new construction.
  • Detailed budgets for proposed new construction.

It may be helpful to obtain expert assistance when writing a business plan. Service Corps of Retired Executives (SCORE) provides such assistance at no cost. Several online resources that may be useful are also available. Sample business plans are available from Bplans; Capital Link has developed a how-to manual called Creating a Business Plan for a Community Health Center Capital Project; and the U.S. Small Business Administration’s Web site includes a “Starting Your Business” section that offers a startup kit with step-by-step instructions for developing a business plan.
