This section describes the organization. It should address the following
topics in one or two pages:
- The clinic’s mission
statement, goals, and objectives (short and long term) and the
goals and objectives of any project described in the business plan.
- A general description of
the clinic.
- Is
it a 501(c)(3)?
- How
was it founded?
- How
long it has been in operation?
- What
is its growth history?
- A brief description of current
and planned programs.
- A description of services
provided.
- A description of the governance
structure (e.g., board of directors).
- The organization’s
overall budget.
